Menopausal syndrome occurs in women around the stopping time of their circle. Now every woman will have the menopausal syndrome. However, for some women, their menopausal syndrome is so severe that they may suffer from hot flash every 30 min with sweat and chilly thereafter the hot flash.

Beside the hot flash, women may have also more or less of headache, migraine, jaw spasm, short of breath, poor sleep, dry mouth, constipation, reduced sex desire, dry and pain in vagina, irritable emotion, anxiety, depression, easy to loss temple, skin rash, dry skin, loss of hair... 

No clear parameter for western medicine to make the diagnosis of menopausal syndrome. For women around 40 and 50 years of old, without evidence of any other reasons for the hot flash, doctors are ready to name your problem as "menopause syndrome".

Menopausal syndrome – How TCM understands it

In Traditional Chinese Medicine (TCM), menopause syndrome is regarded as an imbalance between the Yin Xue and Yang Qi in the body.

Xue, in TCM, refers to the blood, liquid, sweat and material aspects of the body. It belongs to the Yin aspect of the body. Qi refers the bio-power that leads, manages, or governs every function in the body. It represents the Yang aspect. A healthy body has the combination of Yin and Yang. A dead body is pure Yin, no Yang.

For women between the ages of 40 to 50 years, the Yin Xue has been exhausted due to pregnancies, sexual activities, the physical and/or emotional lifestyle. Yang Qi is, in part, transferred from the Yin Xue. When there is  less Yin Xue in the body, there is less Yang Qi to maintain life, physically and emotionally (such as the ability to handle the stress). Therefore, women in menopause feel weakened both physically and emotionally.

Depending upon the clinic manifestation of the syndrome, TCM treatment is variable. Readers can check which group refers to you.  This  helps your doctor make the correct diagnosis. You also need to tell your doctor of any changes you feel when undergoing treatment, since the herbs may need to be changed from time to time. It is not possible to use one herb or one herbal formula to help everyone with the menopause syndrome.  

(1) Emotional disorder mostly - Liver Qi stagnation

The Liver system controls the body’s emotional function, but it is also easily damaged by emotional stress. Anger is the most significant emotional stress that damages the Liver system (constant crying damages the Lung system, constant laughter, the Heart; overwhelming feelings of loss, the Spleen; and feeling overwhelmed by fear, the Kidney). No feeling should be excessive.

People in this group feel, (as do other sub-groups) hot flashes, sweaty, sleep poorly,  irritable, emotional,

reduced desire for sex and eating. Also, there is a bloated feeling in the stomach area, especially in the side of the stomach.

They may feel hoarseness in the throat which needs constant clearing. They can feel sometimes feverish and sometimes shiver with cold. The Doctor observes that the pulse feels like a string of a violin, thin and tight. When touched on the stomach area, the muscle is tender and extremely sensitive. The herbal formula for this group experiencing poor sleep is the Xiaoyao Wan or Jia Wei Xiaoyao wan. It only needs to be taken for about three to five days to feel calm, less tired and reduced hot flashes. If the hot flash remains, we need to work on it with other formulas (see below).

(2) Banxia Xie Xing Tang Zheng

For those with the menopause syndrome having frequent headaches, dizziness, nausea, fear, epilepsy, sleeping problem, or even Jingsheng bing, the herbal formula that contains the herb named Banxia should be considered.

People in this group are special. They feel something wrong in nearly every part of the body. The  feelings are difficult to diagnose and eventually, after several clinic tests, find nothing to be wrong. However, they strongly feel that they are very sick and need care by others. They can even consider that it is time for them to die.

People in this group are very sensitive to physical and emotional stimulations in life. Physically, more sensitive to changes in weather, hot, cold, humidity, noise, light, smell, or taste, etc.. Emotionally, they are very sensitive and are easily hurt by what others say, in anger or in love. They indulged in what they like and escape from what they dislike. Therefore, these persons are often artists, novelists, journalists, designers or whose work involves creativity. They tend to be in blood group AB or A.

They tend to have uneasy feelings on the skin, such as numbness, hurting, burning, tingling, tight, or worms moving. They can feel something in their throat that forces them to clear. In hospital, the doctor cannot find anything wrong. They feel nausea easily and tend to experience car-sickness. Banxia Xie Xing Tang, or the formula with the herb Banxia usually works for this group of people.

(3) Hot flash

For some people, the main problem is the hot flash. The hot feeling comes suddenly and lasts for several seconds or minutes, with or without sweating. Several herbal formula are usually used for this problem.

(3a) Guizhi Tang Zheng.

Guizhi is the name of a herb. Tang, means formula, and  Zheng, a syndrome. The syndrome is a concept in TCM but not in conventional medicine. So, Quizhi Tang Zheng means the clinic syndrome that is suitable to be treated with the Quizhi Tang.

People in this group are usually thin and easily sweat before their menopause age. They do not like to be in the wind since it makes them feel uncomfortable. Their tongue is thin, fresh pink in colour, with little tongue covering. The pulse is floating, little weak or little faster. With the onset of  menopause, it is easy to feel cold after sweating. They usually have less emotional trouble during menopause.

By treating with this formula, any other previous problems, such as skin problems, chronic stomach pain, some types of arthritis, allergic reactions (e.g., asthma), can also be improved. Sweat could be profoundly improved amongst others. 

(3b) Baihu Jia Rensheng Tang Zheng

People in this group have severe hot flashes with a clear dry mouth. A dry mouth is the biggest problem for this group. They also dislike to be in the wind (as does Guizhi Tang Zheng). The hot flash may not be followed by the cold feeling or sweating. They tend to drink a lot of cold water to ease their dry mouth. The pulse feels strong but still kind of weak. These people are usually physically very weak.

This formula works well to reduce hot flashes and the dry mouth within one to two days. (It is very fast!). Along with less hot flashes and a not so dry mouth, one usually has more energy and improved sleep.

(3c) Xiao Chaihu Tang Zheng

People in this group have similar discomfort to those in the Xiaoyao Wan Zheng above, but with clear hot flash-cold feeling or the hot flash which comes at the same time every day, perhaps daily at 3 pm or 7 pm. They may have a bloating feeling in the chest, or their upper stomach and feel tender when touched.. They also tend to have less desire to eat and do not feel hungry.

(3d) Tiao Wei Chengqi Tang Zheng.

People in this group feel hot,  but  often not as a flash. It can last for hours or even all day. They have sweat but the sweat does not reduce the heat. They feel dry in the mouth and are constipated. Their stomach is full and hard to the touch.. Stool may even be felt around the navel. The tongue is very dry with a heavy thick covering, which is usually dark yellow in colour.

(4) Cold feelings

(4a)  Si Ni Sang Zheng

In clinic, there are some people who feel they have very cold hands and feet, but when touched, they do not feel cold. They also have pain, either in their chest or stomach. They may have had this problem for a long time, but they do not look tired physically or emotionally.  The tongue looks “old”, thick, dark red in colour and dry, and the pulse feels strong and string like. The herbal formula recommended is Si Ni Sang.  This is another very popular group. Modern life styles keep people busy and stressed. The Yang Qi in the body is blocked and not well distributed to the extremities (legs, arms).  Often will have a dry mouth and be constipated, for there is  “fire” in the inside of the body. This group will be well treated with the Fen Shui Dan Zheng (see below).

(4b) Fen Shui Dan Zheng

There is another group of people who feel hot flashes, but also have cold hands and feet. Their hands and feet, when touched, are truly cold. Their physical and emotional activities have slowed down, with less energy for daily life and  no career ambitions. They tend to continually nap.  Daytime and night time have

become blurred.. They have less desire for activity, talk very little or are reluctant take on new challenges in life. This group could also have very hot on hands and feet. Especially hot feet, which could wake up them at night. Their hands and feet are cold, especially noticeable at night. The face is pale and the pulse feels weak, deep, thin or even difficult to  locate.

This is a critical situation in TCM. Urgent treatment is needed.  Even if these people feel ok in all other parts of the body, they feel no indication of illness in the body according to conventional medicine. The herbal formula to be considered is the Si Ni Tang, or Fen Shui Dan plus rougui.

(5) Blood stagnation

If none of the above treatments work, we need to check if special conditions exist, which is called blood stagnation syndrome.  This should be considered if the patient has been unwell for a long time but does not seem to be weak physically or emotionally; this condition is very complex and puzzling; their muscles are tight; the tongue is dark red or showing black spots; they have hot and sweaty feeling hands.

Readers should not misunderstand that the above groups are the only groups for menopause syndrome. Clinic condition is very complex. Sometimes one may belong to two or more groups above.
