3b. Mahuang jia Zhu Tang

 

1. 组成

1. Ingredients:

 

麻黄三两,桂枝二两,杏仁七十个,炙甘草一两,白术四两。

Mahuang 45 g, Guizhi 30 g, Xingren 20 g, Zhi Gancao 15 g, and Baizhu 60 g.

 

2. 治疗作用

2. Function:

 《金匮要略痉湿暍病》第20条:湿家,身烦疼,可与麻黄加术汤,发其汗为宜,慎不可以火攻之。

Jin Kui Yao Luo • Jing Shi Die Bing, paragraph 20: For those of patients who are easy to suffer from wetness-related diseases, and if they feel annoying pain in the body, herbal formula Mahuang Jia Zhu Tang can be used. It would be proper to use the sweat-stimulating therapy, not the warming therapy.  

本方用于表实无汗之人, 身体烦痛, 或有鼻塞, 或有浮肿,小便不利等湿气潴留之证.本方证属于西医的纤维肌炎体质健壮无汗者.

This formula is used to the person with body surface-overwhelming (not weak) constitution. The body feels annoying pain, or has stiffness in nose, or has swelling, or difficulty in urination, such wetness accumulation signs. This syndrome can be seen in MS with stronger body constitution and without sweat.  

本方证常见于夏季空调过冷而全身倦怠; 一氧化碳中毒; 洞窟和古井, 深塘, 人多混杂湿气闷热, 郁闭之处; 煤气中毒等毒素作用; 亦可用于流产癖和蓄脓症, 肥厚型鼻炎之头痛和鼻塞.

The condition for the use of the Mahuang Jia Zhu Tang can also be commonly seen in summer when person stays in air-conditioned room for a long time. The person feels tired, kind of nausea, or sluggish. It can also be seen with CO or gas poisoning; an environment with many people and wetness air, such as in bath room, in old cave, or in old well. It can also be used in the treatment of habitual abortion, empyema, or hypertrophic rhinitis, etc.

 

3. 麻黄加术汤用于全身肌肉疼痛, 即纤维肌炎等病症时之方剂鉴别:

3. This formula is pretty good for the treatment of muscle pain in the whole body. The most typical condition of such is multiple sclerosis (MS). When used in such cases, its indication should be distinguished with following herbal formula:

(1). 麻黄加术汤:无汗,身闷痛, 体质较强壮。

(1). Mahuang Jia Zhu Tang: The person feels muscle pain in the body, no sweat. The body constitution is relatively stronger.

(2). 桂枝新加汤: 汗出,身痛剧烈,脉沉迟。

(2). Guizhi Xing Jia Tang: The person feels severe muscle pain and has heavy sweat. The pulse is deep and slow.

(3). 桂枝附子汤: 汗出,身痛,脉浮虚。

(3). Guizhi Fuzi Tang: Muscle pain with seat, chilly. The pulse is floating and weak.

(4). 白术附子汤: 汗出, 身痛, 小便频, 大便偏秘。

(4). Baizhu Fuzi Tang: Muscle pain with sweat. Urination is frequent and stool tends to be hard (kind of constipation).

(5). 甘草附子汤:汗出, 身痛, 擎痛不得屈伸, 小便不利。

(5). Gancao Fuzi Tang: Sweat, muscle pain but more in small joint. The pain could be so severe that the joint is hard to bend. The urination is difficult.

(6). 桂枝去芍加茯苓白术汤: 无汗, 身痛, 小便不利。

(6). Guizhi Qu Shao Jia Fuling Baizhu Tang: Muscle pain, but no sweat. The urination is difficult.

其它有桂枝加术附汤, 葛根加术附汤, 及越婢加术附汤等, 皆为常用之方.

Other formulae also commonly used are Guizhi Jia Zhu Fu Tang, Gegen Jia Zhu Fu Tang and Yuebi Jia Zhu Fu Tang, etc.

 

4. 麻黄加术汤用于急救时之方剂鉴别:

4. Distinguishing diagnosis when use the Mahuang Jia Zhu Tang for emergency conditions:

(1). 麻黄加术汤: 用于气体 (闷热, 恶臭, 一氧化碳沼气, 山峦瘴气等毒气) 中毒后之昏迷.

(1). The Mahuang Jia Zhu Tang is more used in the life threatening condition due to a gas-like poisoning condition, such as CO, terrible smell, choke hot environment, poisoning air in a deep mountains, etc.

(2). 独参汤: 用于体力消耗过度导致极度气虚, 或大出血后气随血泄等消耗性休克状态. 表现为面色苍白, 手脚冰凉, 血压下降, 冷汗淋漓、气息微弱、脉微欲绝.

(2). Single Shen Tang: This formula contains only one herbal ingredient: Ginseng. It is used in extremely Qi deficiency due to over-exhausting of body energy; or in exhausting condition due to heavy loss of blood, in which the Qi escapes along with the loss of blood. It is kind of shock condition. The person shows pale in face, cold in hands and feet, cold sweat, weak breath, very weak pulse and very low blood pressure. 

(3). 吴茱萸汤: 少阴病,吐利,手足逆冷,烦躁欲死者。

(3). Wuzhuyu Tang: It is used in Shaoyin stage of Shanghan disease. The patient has vomit, diarrhea, cold hand and feet, and feels annoying to die. (This condition can be easily seen in acute bacillary dysentery.

(4). 通脉四逆汤: 少阴病,下利清谷, 里寒外热,手足厥逆,脉微欲绝,身反不恶寒,其人面色赤,或腹痛,或干呕,或咽痛,或利止脉不出者。

(4). Tong Mai Si Ni Tang: It is used in Shaoyin stage of Shanghan disease. The patient has diarrhea with non-digested food in stool, cold hands and feet. The pulse is very weak or it is hard to feel. The body however does not dislike cold. The face is red in color. The person may have pain in stomach, or nausea, or sour throat, or the diarrhea stops but the pulse remains hard to feel.

(5). 白通加猪胆汁汤: 少阴病, 利不止,厥逆无脉,干呕烦者。

(5). Bai Tong Tang Jia Zhu Danzhi Tang: It is also used in Shaoyin stage of Shanghan disease. The diarrhea continues; the patient has cold hands and feet; the pulse is very difficult to feel; and the patient has nausea and feels annoying.

(6). 大承气汤: 少阴病,自利清水,色纯青,心下必痛,口干燥者。少阴病六七日,腹胀不大便者.

(6). Da Cheng Qi Tang: In the Saoyin stage of Shanghan disease, the patient has diarrhea as clear water, which can be kind of blue in tint. There must be a pain in upper stomach. The patient feels dry in mouth. The formula is also used in Shaoyin stage when a patient feels bloating in stomach and has no bowel movement.
